Parishes/congregations that decide to sponsor this ministry submit this form.

Persons in leadership positions complete and submit this form.  This includes parish/congregation steering committee members, leaders and co-leaders of presentations/events, board of directors, regional advisory council, regional coordinating groups, and program committees.

Program Notification form
This form is submitted for each program presentation or event to announce and advertise the event on this ministry website.

Program Summary form
This form is submitted following each program presentation or event.
